  100% due at Inception
• Three Pay
  1/3 due at Inception
  1/3 due at 30 days
  1/3 due at 60 days
• Ten Pay (EFT)
  20% due at Inception
  Balance due in 9 equal installments
  Interest free.
• Ten Pay (Cash)
  20% due at Inception
  Balance due in 9 equal installments
  Interest charge 9% of balance due.
• Quarterly Pay
  35% due at Inception
  25% due first day of 2nd Quarter
  25% due first day of 3rd Quarter
  15% due first day of 4th Quarter
  Interest charge 9% of balance due.
• Monthly Pay
  20% due at Inception
  Balance due in 9 equal installments
  Interest charge 9% of balance due.

A. Coverage becomes effective when payment is received in our office. Past due policies greater than 10 days are subject to cancellation.
